Overview
Your first visit with PhysiciansCenter of Physical Medicine involves you and your physician getting to know each other. It is important to fill out your paperwork entirely and share all information available with your physician and his medical assistant.

Consultation
To maximize efficiency please come to your office appointment with your insurance information and patient questionaire completed. We also request that you have a drivers license or photo ID. Please present these at the check in window when you arrive. Patient gowns are provided for the physical examination. Please come appropriately dressed.
Electromyography and Nerve Conduction Studies (EMG/NCS)
EMG/NCS is basically a test of muscle and nerve function. The test involves electrical stimulation of peripheral nerves to determine their health. A small shock will be administered during this test and is generally well tolerated. The second part of the test is called the EMG. This part of the test involves examination of muscle function using a small accupuncture size pin. This pin picks up small electrical impulses from the muscle and is analyzed by the physician performing the test.
Insurance and Billing
Our practice accepts all major insurance carriers including Workman's Comp. We will work with your insurance company to receive payment. When you come to your first appointment please provide us with your insurance card or insurance information. We do ask that you pay for any co-payments or deductibles that your insurance requires at the time of service. We accept Visa, MasterCard, and personal checks for your convenience.
Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own physical therapist.
